1.2b14: assertion failures..

These are fairly frequent (5 in 24 hours)

squid: store_swapin.c:30: storeSwapInStart: Assertion `e->swap_file_number >= 0' failed.

Haven't yet tracked down where they're coming from. Anyone know
anything obvious?
