 I don't think its to do with disk io, unless your box is in heavy paging.
 "irregular intervals" =~ "real life"? Maybe your cache is having bursts
 of incoming connections? you know that squid gives highest priority to
 incoming sockets, including ICP. ALthough 3 secs delays seems very high.

 Maybe your dnsservers are blocking?
