Design puzzles:

1. Why does Squid choose periodic removal policy over need-based removal
policy?

2. What are the advantages of keeping the size of cache within the
lower_water_mark and the upper_water_mark?

3. Is it possible to sort a hash table? Why don't you try to remove the
storeEntry with the oldest timestamp first in storeMaintainSwapSpace?

Other questions:

1. How do you know a request is a hit?

2. Where do you set a storeEntry is both fresh and in the cache? (I want
the function name.)
