Closed SourceForge branches

The "modio" and "cygwin" branches on SourceForge has been closed in
their current forms as they has been fully integrated into Squid-2.4.

If needed then the branch names can be reopened again by using the
cvsmkbranch script to create a new branch with the same name, and then
editing projects.html to indicate that the branch is indeed open again.

Any old remains in the branches which did still exists after the merging
is available as diffs downloadable from
http://squid.sourceforge.net/projects.html
