NTLM update

Several bugs in NTLM authentication when dealing with untrusted
domains, wrong passwords, helper arguments and more have been fixed in the
auth_rewrite branch and are waiting for merge into HEAD.

Guinea pigs, err... early adopters are strongly encouraged to
update and report their successes (failure is not an option :-)
