new branch - diskio

Hi,

After all the discussion we've had on this list covering
the need to abstract the FS and IO out a little bit,
I've decided to actually do it.

the branch name is 'diskio'. The target is to tidy up the
existing code in src/fs/, which includes abstracting the disk
IO back out into modules a la 1.x and early 2.x, unifying
ufs/aufs/diskd into 'ufs', and fixing COSS.

COSS now seems to work again in sync mode. Yay.
