Cygwin lacks icmp support - pinger.c PATCH

Hi,

Today I discovered why nobody asked before to fix the pinger.exe build
problem on Cygwin:

Simply, Cygwin lacks include files for icmp: the netinet/ip_icmp.h files is
empty ....

So I have just adapted the Windows native port changes in pinger.c to
compile and run on Cygwin too.

In the attached patch file there is a full Windows update of pinger.c,
supporting both _SQUID_CYGWIN_ and _SQUID_MSWIN_ environments.

IMO this can go in HEAD and 2.5.
