squid3: forward state memory leak

hi,

I'm polygraphing squid3 at the moment and I've noticed a
memory leak in forward.cc .

the fwdState for a connection isn't being freed as the majority of
connections have a single reference somewhere to it. A few have
multiple (9,10,11) references but the majority have one.
