It should be a Windows only problem of cachemgr.cgi:

Using a Linux cachemgr.cgi, the resulting error page is fine from both
Windows and Linux Squid versions, Using a Windows cachemgr.cgi, the
resulting error page is wrong from both Windows and Linux Squid versions.
