update: half-closed

Here's the latest diff which, from my testing, implements
Henrik's listed behaviour for half-closed and normal
connections, pipelined or not.

I've turned on prefetching and it also seems to work.

Can I have some testing? I'd like to commit this over the weekend
and move onto more strange bugs.
