knowledge base stuff

Something I've been meaning to do for a while is assemble a knowledge base of
common specific problems. Kind of like the FAQ, but less "how do I do this?"
and more "It broke like X, how do I fix it?"

It might get merged into the FAQ, I'm not sure at this stage.

I'll start populating it as I see squid-users posts w/ solutions that I think
merit recording.
