Looks good.

There is a couple questions, but nothing major.

For Squid-2 please use Squid-2.HEAD and not 2.6 contrary to what I said
before. There is some 2.HEAD cleanups not yet in 2.6 or 3.. but it's ok
to wait with that for a while.

This is intentionally documented together with udp_outgoing_address as

Think the Squid-3 formatting is preferred here. Leaves more room for the
description. And also keeps the style of http_port and https_port the
same..
