write-side delay pools; per-connection shaping

I've finished the write side delay pools and per-connection shaping
(which I've migrated to class '6' to not clash with Squid-3.)

The diff can be found at

I'd like to commit it to Squid-2.HEAD in the next couple of days. I'll
see if I can separate out the write-side delay pool and the new class
and do them as two commits.
