NTLM IP caching code in Squid 3.0 ?

Hello all:

I have been having many issues with NTLM authentication popups appearing
randomly when running Squid. They seem to happen somewhat randomly, but
I can trigger them ~80% of the time by going to a particular website and
trying to upload pictures, etc.

I am currently using:
Squid 3.0.8
Samba 3.0.31
Gentoo Linux
Samba is configured to be a member server of a Win 2k3 Active Directory
domain.

The Clients are almost exclusively IE7 on Windows XP.

I can upgrade squid and/or samba if there has been a fix to fix my
issue, but I did not see one in the change-logs (for Squid anyway...)

The common fix for this appears to be using the NTLM Ip caching patch,
which I found hosted on: http://devel.squid-cache.org/projects.html
However, this has not been ported to Squid 3.0 that I have been able to
find.

I can port this patch myself, however I would not like to duplicate
someone else's effort if it has already been done, and I will need some
pointers, as I am new to the codebase.

If anyone has any suggestions as to what may be causing the problem, I
can provide logs and debugging output as requested.
