The BNF requirements on query-string are:

   query = *( pchar / "/" / "?" )

   pchar = unreserved / pct-encoded / sub-delims / ":" / "@"

   unreserved = ALPHA / DIGIT / "-" / "." / "_" / "~"

   sub-delims = "!" / "$" / "&" / "'" / "(" / ")"
                 / "*" / "+" / "," / ";" / "="

'|' characters is only permitted when %-encoded.

Squid is expected to deliver properly %-encoded URLs to helpers, logs,
and any outgoing messages regardless of how it was received.
