I've now had a crash which did leave traces in the cache.log:

Interestingly the cache reload went at a sensible rate this time, since
Squid wrote out it's logfile cleanly. I guess the slow rate I've been
seeing is because on the other failures it hasn't been doing that, and so
a slow rebuild has been forced.
