Constant DNS Fails

Hi Squiders,

I am still having this dns fail error happening of late, and the squid
needs to be terminated and restarted

Does anyone have any idea to the cause of this constant PIA

squid 1.1.8
DNS_server 10 (5 used, 5 unused)
linux machine (Slackware 2.0.0)
P100 CPU
128MB Ram
6GB Cache (Uses approx 75% MEM)
etc,etc
The machines CPU only gets used for 1%
Utilisation 42%-52%
