There's probably something sick in your passwd file - tried checking with
something like

  grep -c ..*:..* file
  wc -l file

making sure they are the same, and so on? You could just add back the
lines 50 at a time until you find the batch with the bad one.

We run squid 1.1.15 + acl proxy_auth + the security fix now in 1.1.16
quite happily with ~10,000 passwd entries.

Of course, if yours was working with 1.1.15 there's some new bug, but you
didn't mention that it has been working in the past...
