Given that you mention FreeBSD, the Adaptec aic7xxx based devices are the
controller of choice. Others like the Buslogic family etc, but the FreeBSD
support for the Adaptec devices is orders of magnitudes better than the
others at the moment.

For FreeBSD, your best choices would be a DEC 2114x based card (eg: SMC
9332BDT etc), or the Intel Etherexpress Pro/100B type cards.
