New disk storage scheme for improved performance

On November 10th, Andres Kroonmaa mentioned in a mail to the Squid
developers list that the current storage scheme of Squid was far from
optimal because it does not make optimal use of the Unix file system
cache. I made a patch which tries to solve the problem.
