What OS are you running? If you're running BSDI, try lowering the
cache_mem value a LOT and then lowering the high and low cache_mem
watermakrs. I use about 30% for low and 43% for high. Works for me :-)

Of course I found a better solution: I switched my caches to linux...
Squid seems to run a lot better on Linux than on BSDI.
