I've setup it this way:

acl manager proto cache_object
acl localhost src 127.0.0.1/255.255.255.255
http_access deny manager !localhost

So, ACL named 'manager' denied for all except localhost. proxy_auth
is not used here. Why Squid anyway requires proxy_auth
for cacgemgr.cgi ?
