I had big boost from using dlmalloc (there's a link to it on
http://squid.nlanr.net). Something that paid off big too is
doing asynchronous meta data updates on the filesystem, and
in src/disk.c (squid 1.1.21) doing an fsync() on the file
descriptor before it is closed (in file_close()). Also have
atime updates turned off.
