Assuming you use the 1.2 Squid with the external proxy auth patch, you
can use the NCSA auth program as a really straightforward model to doing
this. Or, better yet, write a really simple Perl program instead of C.

More info on the patch:

Subject: DBM Authentication

Has anyone written a module for squid to authenticate off of a DBM file?
If not how do I go about writing a module for squid. (i.e Where do I
tell
squid to use that module etc) Thanks.

I liked things better when I didn't understand them.
