Try these rules instead:

rdr ef0 202.150.2.12/32 port 80 -> 127.0.0.1 port 80 tcp
rdr ef0 0.0.0.0/0 port 80 -> 127.0.0.1 port 3128 tcp

(The first rule is to prevent squid from connecting to itself.)

httpd_accel_host virtual
