I was finding that we were getting this on our caches, when the reverse
lookup of the proxy server didn't match the forward lookup.

eg... we run our squids with multiple IP's to utilise different links.

When using 203.33.199.213, 203.33.199.213 resolved to
proxy2.per.wantree.com.au.

But proxy2.per.wantree.com.au resolved to 203.63.10.9, which caused an ip
mismatch (and it seems some servers are pedantic about that [netscape
server was one that kept rejecting us and giving document contains no
data.]).

http://www.westpac.com.au is one example that springs to mind that
requires reverse and forward lookups to match.
