What are the status code of these TCP_DENIED? If it is TCP_DENIED/407
then it is Squid asking the user to authenticate. Such log entries can
most of the time be ignored. If you look closesly at the log you will
notice that it is usually followed shortly by another access which is
sucessful (i.e. after the user has authenticated themselves with their
username and password, or the browser understood that it has to send
authentication credentials to the proxy).

If if is TCP_DENIED/403 then you have an ACL which blocks the request.
