Squid user login limit

Hello squid-users,

I write a patch to implement login-limit on Squid.
This disable the user access from more than one ip address at the same
time, inside the authenticate_ttl time.
Login limit require PROXY_AUTH utilization. This patch is in test, use
at your own risk.
I am running in test environment with 20 users, with no problem at the
moment.
