The remaining questions are: (I know you have answered some already, but
I did not write down your answers)

* Is the time on your proxy server reasonably correct?

* What refresh_pattern settings are you using?

* Can you enable log_mime_hdrs in squid.conf, and then send the output
of one "failing" request from acccess.log and store.log.

* What is your maximum_object_size setting?
