I have a feeling that this is occurs when Squid has read in a lot of data
for a large request that has not been completely delivered to the client
yet - it only occurs for the byte ratio, not the request ratio, and it
seems to occur when I'm downloading very large files. I'm sure someone
will correct me if this is a false assumption...
