help needed

i am running squid 2.2Stable4 on Redhat 6.1
 
i am frequently observeing following things

from cachemgr.cgi -> general runtime information -> median service time

1> cache misses 5-6 seconds sometimes even grater then 7 seconds.
2> near hits 2-3 seconds sometimes even grater then 4 seconds.

what exactly is 1> cache misses
                        2> near hits

one another problem is that
when i give

free command is showing

total space = 582 Mb
 used = 578 Mb
squid is using only 140 MB

1> buffer space = 380 MB -> for what this space is used for it's growing rapidly as no. of requests increases
2> free space =3 Mb
