ncsa auth module

I am trying to setup proxy authentication. The first step appears to be to
setup the ncsa_auth module. However I installed Squid from an rpm and
don't seem to have the auth_module/NCSA directory mentioned in the FAQ.
Have been following links all afternoon, so now where do I get a copy of this
module?
