Patch: have NTLM authentication work

This patch against Squid-NTLM from CVS fixes a few buglets which prevent
it from working (sorry, a few indentation changes seeped in too):
-the order of the Proxy-Authenticate headers appears to be relevant
-size -> sizeof in carpSelectParent
-need some ident-related fields in structs.h
-squid was forgetting the auth-type halfway through the authentication
sequence

It _still_ doesn't work well enough, as it bombs while trying to retrieve
the
password from the challenge if we remove the STATIC_CHALLENGE define in
authenticate.c.
Is anybody willing to investigate and try to solve this with me?
