So you are running a "transparent" proxy, and thereby violating the
end-to-end awareness assumption in TCP/IP and related protocols.

The reload button are supposed to always force a revalidation of the
cache, and Shift+Reload (or is it Control+Reload in IE?) are supposed to
force a complete refetch of the page.

Due to some implementation issues (correct, but different) IE does not
indicate the use of the reload button to caches unless configured to use
a proxy, while netscape always indicates this regardless if it is
configured to use a proxy or not.
