ldap_auth interfaces with LDAP, ignoring PAM.

If the helper works from the command line then any configuration of the
helper is completed and it should work from Squid.

What do you get in cache.log when Squid starts?

Are you positively sure you have specified authenticate_program
correctly in squid.conf?

Quick check:
  squid -k parse
