You need the following line near the top of your MRTG .cfg file:

LoadMIBs: /usr/share/snmp/mibs/SQUID.txt

or if you're running on NT you'll have to adjust the path to suit the
location of the mib file. It comes with the squid source distro as
mib.txt.
