ok, thanks to all. Now I have the form with:
  Cache Host:
  Cache Port:
  Manager name:
  Password:

I have changed squid.conf to include :

acl mywwwhost src a.b.c.d/q.w.e.r where a.b.c.d and q.w.e.r are
host and netmask respectively

The rest is more or less as FAQ suggests

I get this message:

connect: (111) Connection refused

What ' s the following step ?

Thanks in advance
