These are used on replies not having a explicit expiry time set by the
origin server.

In theory min is the minimum age a object without a known expiry time is
considered fresh and max is the maximum age. In between there the
percentage value is uses to calculate a freshness interval based on the
modification age of the object.

In truth however it is not exacly like that (the min age is a little bit
different than the theory), and there are many refresh_pattern options
to change the algorithm..
