I noticed something... That third copy *isn't* coming from the squid mail
server, but from some other server. If you look at the bottom of the
Received headers, you'll see:
Received: from squid-cache.org (squid-cache.org [206.168.0.9])
 by www.d-ns.net (8.9.3/8.9.3) with SMTP id EAA13627
 for <arthurtan@jurongcement.com>; Wed, 23 May 2001 04:44:43 +0800
... Perhaps <arthurtan@jurongcement.com> needs to be contacted and/or
removed from the list?
