but df -h shows
/dev/md4 3.8G 2.8G 874M 77% /cache

What next?

Tom,
have a look in /var/log/squid/<squidname>-cache.log for any
errors; if there's nothing helpful then try to start it in debug mode
(squid -XNC). Check for anything daft like another server running
on your squid port.
