Odd.. but you say you are using Squid-2.3.. if so have you compiled it
with --disable-internal-dns? (if not, /etc/hosts is ignored and
Squid-2.3 only uses DNS). I guess you have as it seems some requests
works, and it only hangs on the requests where you see the supposedly
harmless warning "no data to process" in cache.log.

I would recommend using the DNS registered FQDN of the server, or an IP
address. But I am quite unsure on what is going on here. If hostname
resolution was the problem nothing should work. Can you please try
upgrading to a more current Squid version such as 2.4.STABLE2?

Hint on iptables: what you are looking for is done by using DNAT.
Requires IP forwarding to be enabled and return traffic from the server
to be routed via the iptables box. (the latter can be forced by also
using SNAT, but then hides the clients real IP from your server).
