By using the urlpath_regex (or url_regex) ACL type.

However, this only looks at the URL, so if a site uses .exe for CGI
programs on a Windows server then these will most likely also match.
What you can do there is to add a second list of exclusions which are
allowed even thou it looks like an exe file..

In the upcoming Squid-2.5 release you have additional tools to block by
content-type, not only URL.
