This sounds like a routing or networking problem--not a Squid problem,
unless you have altered your refresh pattern in such a way that it
overrides no cache headers.

Squid will, in some cases, serve stale content rather than no
content--so if your Squid can't reach the site anymore, or it has
extremely long timeouts it may serve the old pages. Most likely if you
fix the network problems, your 'squid problems' will disappear.
