Next time it happens, reconfigure squid rather than stopping. If that
fixes it, we can check the code in that light.
If it doesn't, edit the config file, and up the debugging level for
client_side and acl's (uhmm, from memory thats 28 and 33 - but see
docs/sections.txt) to 5 or 6 and then reconfigure again. This will raise
the amount of data logged and will include what acl is causing the deny.

You can also edit your error pages to include the name of the acl that
caused denied access.
