Authenticate_program /usr/lib/squid/NCSA /etc/passwd
 
to create a passwd file you need htpasswd from apache.
 
cmd line is - htpasswd -c passwd useraccount

So it will be this I guess;
 
Authenticate_program /usr/lib/squid/pam_auth /etc/passwd
