Seems to me that a what easier approach would be to have a specialised NAT in front of the proxy farm that maps to destination, not by source.

I.e. given a pool of n ip address's the NAT device assigns 1 ip (bar) for the first instance of a connection to external ip foo. Every proxy member, regardless of ip, gets mapped to bar for the lifetime of the translation.

Some hacking might be needed, but IMO it's better because
a) it's simpler
b) full use of the stateless any-any failover you have within the two layer farm is possible without disrupting user state.
c) Most (all?) Web sites support multiple users from one ip, because web proxies are well known. (Just web proxy farms are more rare).
