These are the name of the host and theport that squid runs on. If your web
server and squid are on the same box, fine. If not, put the name of the
host in the box. If squid isn't on port 3128, put the right number in the
box.

> pws : *****

Have you set the name and password in squid.conf? If not leave them empty.
