By analyzeing those last 3% and figure out why they were not hits. Then
decide if you really want to make them hits, and if decided, figure out
how to make Squid see them as hits.

Enabling log_mime_hdrs is a good start, then look for
Expires/Pragma/Cache-Control in the requests and replies (see RFC2616
for their meaing).
