Thanks. I've got it working now, but only by guessing on the use of
auth_param, based on
http://ftp.pop-mg.rnp.br/squid/mail-archive/squid-users/200210/0004.html

Does anyone know where to find squid v2.5 documentation? Of course it's
largely the same as 2.4, but this is a case in point where 2.4
documentation will not help.
