Check your log files. if cache.log doesn't show the helpers starting,
then you have a config problem. If you see a 'ignoring proxy auth
because no auth schemes configured' move the acl's lower in the file
(after all the auth_param statements). Make sure that you have no ip
based http_access statements that would allow access before the proxy
auth one.
